Título: Muscular strength and vibration thresholds during two years after acute poisoning with organophosphate insecticides.

Abstract: To evaluate the association of acute organophosphate (OP) poisoning with chronic sensory and motor neurological impairment. METHODS: This study concerns the third of a series of three examinations of hand strength and vibration thresholds in a two year period after acute OP poisoning among 48 Nicaraguan men. The first two examinations were performed at hospital discharge and seven weeks after poisoning, and the present examination two years later. Twenty eight cattle ranchers and fishermen who had never experienced pesticide poisoning were examined as controls, also three times over the two year period. The poisonings were categorised as caused by "non-neuropathic" OPs and "neuropathic" OPs, each subdivided in moderate and severe poisonings. RESULTS: Men poisoned with OP insecticides had persistent reduced hand strength. We previously reported weakness at hospital discharge for OP poisoned in all categories that worsened seven weeks later for those severely poisoned with neuropathic OPs. Strength improved over time, but the poisoned were still weaker than controls two years after the poisoning, most noticeably among the subjects most severely poisoned with neuropathic OPs. Also, index finger and toe vibration thresholds were slightly increased at the end of the two year period, among men with OP poisonings in all categories, but patterns of onset and evolvement of impairment of vibration sensitivity were less clear than with grip and pinch strength. CONCLUSIONS: Persistent, mainly motor, impairment of the peripheral nervous system was found in men two years after OP poisoning, in particular in severe occupational and intentional poisonings with neuropathic OPs. This finding is possibly due to remaining organophosphate induced delayed polyneuropathy.

Abstract: En esta NTP, continuación de la 512, se describen los principales tipos de efectos tóxicos de los organofosforados en distintas situaciones de exposición, su relación con los posibles trastornos (manifestaciones clínicas) a que pueden dar lugar y los aspectos fundamentales de algunos de los mecanismos de acción aplicados a los trabajadores expuestos. Además de los efectos agudos, con sus manifestaciones muscarínicas, nicotínicas y del sistema nervioso central, describe los efectos retardados y se refiere a la exposición repetida o crónica con sus efectos permanentes y otros efectos neuropsicológicos. También se exponen algunos de los requerimientos para el diagnóstico etiológico en caso de sospecha de exposición aguda, así como sus limitaciones..

Resumen


El objetivo del presente trabajo es revisar críticamente la forma en la cual se lleva a cabo la vigilancia epidemiológica de los efectos sobre la salud aso­ciados con la exposición a plaguicidas en Colombia. Para tal fin, primero se describen los cambios históricos en la exposición a los plaguicidas en el país, se plantean los esperados y potenciales efectos adversos sobre la salud (crónicos, agudos y sub-agudos). Luego se revisan algunos de los sistemas de vigilancia epidemiológica existentes en el país, se analizan sus fortalezas y debilidades, y por último se proponen algunas pautas básicas para mejorar dicha labor.

Abstract

Surveillance of pesticide poisoning in Colombia

The objective of this article is to realize a critical review of the epidemiologic surveillance of pesticide-related health effects in Colombia. First, it describes  the historic changes in the exposure to pesticides in the country, the expected and potential adverse health effects (chronic, acute, and sub-acute). Second, it reviews some epidemiologic surveillance systems actually used in Colombia with their strongholds and weakness. Finally, it proposes some basic issues to improve this important public health activity.

Título: Risk factors to pesticide exposure and associated health symptoms among cut-flower farmers.

Abstract: The study looked into the risk factors associated with pesticide exposure among cut-flower farmers. A survey questionnaire was given to 102 respondents in Barangay Bahong in La Trinidad, the center of cut-flower production in the Philippines. Results showed that 32% were symptomatic or had experienced pesticide-related illnesses since their first use of pesticides. The majority of the pesticides used by the farmers were Categories Ib and II which are moderately or highly hazardous chemicals. Individuals with signs and symptoms most often centered on the eye, ear, nose and throat (EENT) (44 respondents reporting these

symptoms) followed by general and neuralgic (16 respondents) and the integumentary (14 respondents). The most common general signs and symptoms manifested were weakness followed by fatigue and muscle pain then by chills and fever. The most common EENT manifestations were eye itchiness and blurring of vision. For neurological signs and symptoms, dizziness followed by headache was reported. Logistic regression showed that illnesses for the past 12 months were associated with certain risk factors such as farm use of pesticides, exposure to pesticide while applying it, respiratory inhalation of pesticide vapours and mists (p = 0.05). Moreover, those who re-entered a recently sprayed area were 20 times more likely to get ill during the past 12 months than those who did not. Those who used pesticide-contaminated pieces of fabric to wipe sweat off their faces were 2% more likely to get ill, and those who had spills on their bodies while applying pesticide were 26 times more likely to get ill. The study suggested that the risk factors to pesticide exposure should be considered in policy formulations for the cut-flower farmers in the country.

Abstract: Information note for occupational physicians. Allergic eczema due to agricultural pesticides is seen mainly among farmers, gardeners and florists. The following products are discussed: fungicides (anthraquinones, imidazoles, phthalimides, thiram and dithiocarbabmates, chlorothalonil, quintozene), herbicides (nitrofen, nitralin), insecticides and acaricides (hexachlorocyclohexane, organophosphorus compounds, propargite, 2,4-dinitrophenylthiocyanate, dinobuton), nematocides (metam sodium, dazomet) and rodenticides (antu). A test battery of agrochemical allergens is given. Diagnosis, prevention and compensation are discussed.

Abstract - In this study, 150 workers exposed to pesticides and 50 healthy control subjects were given clinical and dermatological examinations, patch tests, tests of liver and renal function, complete blood count, blood sugar and urinalysis. Activity of the antioxidant enzymes superoxide dismutase, glutathione peroxidase and glutathione reductase was also evaluated. Dermatological findings were positive in 78%, 76% and 54% of workers exposed to organophosphates, pyrethroids and carbamate pesticides respectively. The patch test was positive in 70% of workers exposed to pyrethroids and 64% exposed to carbamate pesticides.
